Requirements
Care Level 5 is granted when a person exhibits "the most severe impairment of independence" requiring special nursing support. The care level is determined based on a care assessment, for which at least 90 points must be achieved. This assessment evaluates the extent of a person's independence and replaces the previously decisive criterion of care expenditure.
To receive Care Level 5, you must submit an application for an upgrade to your long-term care insurance. They will arrange for a new care assessment, during which an expert will evaluate the situation. The care assessment awards up to 100 points, which are comprised of six different areas. There are exceptions, for example in childcare or special needs situations.
Benefits for Care Allowance Level 5
People with care degree 5 require intensive round-the-clock care and support. To ensure this care, comprehensive support from long-term care insurance is necessary. With care degree 5, you are entitled to all care services in full, provided the respective requirements are met.
Compared to care degrees 2, 3, and 4, you now benefit from extended services, such as higher care allowance, extended in-kind care services, as well as extended day and night care and inpatient care (e.g., nursing home). Other services remain unchanged.

Home care for care level 5
Home care for care level 5 is a major challenge that can hardly be managed without careful planning and professional support. It is crucial that caregivers do not overexert themselves. Nursing care benefits for care level 5
If you organize home care independently, you will receive care allowance of 990 Euros per month. If you also use care services in kind, the amount will be reduced proportionally.
No cost receipts are required for the care allowance, but you must participate quarterly in a free consultation according to Paragraph 37.3, where you can ask individual questions about care.
Inpatient care for care level 5
In the case of care level 5, the long-term care insurance covers 2,096 Euros per month for inpatient care. However, the out-of-pocket contribution remains the same, regardless of the care level, and is based on the "facility-uniform own contribution" (EEE).
The total costs consist of care costs as well as expenses for accommodation, food, and investments in the facility, which vary depending on the nursing home.
Care benefits in kind
Care in kind services allow you to engage professional carers for home care, with the care service being able to bill your long-term care insurance directly.
With care level 5, you have 2,299 Euros available monthly. Please note that your entitlement to nursing care benefits will be reduced if you use care in kind services.
Combined Benefit
With the combination benefit, you can combine in-kind care benefits and care allowance as needed, for example, 75 percent in-kind benefits and 25 percent care allowance. You can flexibly adjust the ratio of in-kind to cash benefits to your individual situation.
This option is particularly useful if only partial professional care is required and the in-kind benefit amount is quickly used up. It thus offers you more flexibility in organizing care.
Respite care
Sometimes caregivers also need a break for appointments, illness or vacation. In such cases, substitute care allows for hourly or daily representation for up to 42 days or 6 weeks per year.
1,685 Euros are available per year, which can be used for representation by private individuals or an outpatient care service. Unused funds from short-term care can also be partially used for substitute care.
Short-term care for care level 5
Short-term care offers temporary inpatient care for people who are normally cared for at home. It is often used when health temporarily deteriorates.
For short-term care, an amount of 1,854 euros per year is available, with a maximum duration of 56 days (eight weeks). Unused funds can also be partially used for respite care and vice versa.
